Position Summary:
The Reentry Navigator provides comprehensive, person-centered reentry services for individuals incarcerated in the Jackson County Jail, to include individuals with opioid use disorder (OUD), other substance use disorders (SUD), co-occurring mental health needs, or overdose risk factors. The Navigator conducts pre-release assessments, develops individualized reentry plans, facilitates warm handoffs, and coordinates access to medications for opioid use disorder (MOUD). This role includes overdose prevention education, naloxone distribution, and collaboration with jail, medical, and community partners.
