主祀神明: 池府千歲
主祀神明的稱呼: 池府大千歲、池府二千歲、池府三千歲，陪祀神明:桐王、玄天上帝、天上聖母、九天司命真君、土地公。創立於乾隆初年間，約1735至1740年，由本庄先輩賴錠公獻地創廟。
