• 5 Copies of Birth Certificate
• 5 Copies of Marriage Certificate (for married female applicants)
• 3 Copies of Form 137/138 or Diploma (if applicable)
• 4 Passport-Size Pictures (TESDA Standard)
• 3 Pieces 1x1 Pictures
• 1 Long Size Brown Envelope
