COLLEGE EDUCATIONAL ASSISTANCE!!
(2nd Semester S.Y. 2025-2026)

Registration Time: 1:00 PM
Follow the steps below to complete your registration.
1. To register, Scan the QR code or click this link. (https://citizen.trecemartirescity.gov.ph/EducAssistanceRegistration.aspx?fbclid=IwY2xjawMvbWhleHRuA2FlbQIxMABicmlkETFmZGZlNEhvNnBMUEFBZFZ5AR6yTeq6IOL7hViy3CV1lWFbVzhFXE-FIEQ6Oph_-M1hMxiUEREAo2aOnE73bQ_aem_Y24OcMahHri2Y-MlyF988g).
2. Confirmation and submission details will be sent via email address.
3. Submit your requirements to the Office of the City Social Welfare and Development Officer on the scheduled date between 8:00 AM. and 5:00 PM. Ensure all documents are enclosed in a long plastic envelope.
List of Requirement/s:
•Two (2) copies of the fully accomplished General Intake Sheet form with 2x2 picture, downloadable via email address or click this link (https://docs.google.com/spreadsheets/d/1GaqTe2DX14AI1HO70cR29yXILssdDo9-/edit?usp=sharing&ouid=110297138209249738236&rtpof=true&sd=true ).
•Letter addressed to the: City Mayor, Hon. Gemma Buendia-Lubigan.
•Certificate of Registration form with Original Dry Seal or Certified True Copy (2nd Semester SY. 2025-2026).
•Original Barangay Clearance (Purpose: Educational Assistance).
•Photocopy of Voter’s Certification or Voter’s ID.
•Photocopy of Valid ID.
4. The Office of the City Social Welfare and Development Officer shall undertake a comprehensive evaluation and verification process to ensure the validity and authenticity of all submitted documents.
5. A list of qualified students will be posted/announced, followed by a notification regarding the College Educational Assistance distribution details. Please stay tuned for further updates. ‼️‼️
REMINDERS:
• Submit your documents on your confirmed schedule appointment ONLY!
• All applicants for College Educational Assistance are still subject for screening;
• Students with incomplete requirements will not be entertained;
• ONE DAY Registration, FIRST COME, FIRST SERVE BASIS;
• Student without online schedule may visit OCSWDO for waitlist;
• ONE Beneficiary per family ONLY;
• 2000 Beneficiary ONLY;
• The Office of the City Social Welfare and Development Officer will personally validate the authenticity of your documents to your school registrar.
