4. Affix a P30.00 Documentary Stamp;

5. Attach a copy of PSA or LCR-issued Birth Certificate (either Original, Certified True Copy, Printed Copy, or Photocopy) to ALL copies of the COC

6. COC must be notarized/under oath and bear the seal and signature of the Notary Public

7. BIO Data (OPTIONAL)

8. All copies must bear the candidate's original signature
