✅ Accomplished Enrollment Form
✅ Report Card (SF9)
✅ Good Moral Certificate
✅ Photocopy of Diploma
✅ Photocopy of PSA Birth Certificate
✅ Photocopy of National ID (or National ID Slip/Receipt)
✅ Two 2x2 ID Picture
✅ Down Payment

📌 For Grade 11 students from Private Schools:
Please submit your ESC or OVAP Certificate.
