The Persons with Disabilities Affairs Office (PDAO) Taguig is inviting qualified couples to participate in the Kasalan Bayan for Persons with Disabilities, scheduled for:

📅 September 26, 2026
📍 Venue: To be announced

QUALIFICATIONS UNDER ARTICLE 34

The couple must:

🏠 Be living or staying in the Philippines and have been residing together in the same household continuously from the start of their cohabitation up to the present; and

💑 Both parties must be single.

Note: Article 34 does not apply to couples who are divorced, annulled, or widowed for less than six (6) years.

REQUIREMENTS

📌 Four (4) copies of the Affidavit of Cohabitation from a Notary Public, signed by both parties
📌 PSA Birth Certificate of both parties
📌 CENOMAR of both parties
📌 Valid ID
📌 Birth Certificate of child/children, if applicable

• Ang PhilHealth YAKAP (Yaman Ng Kalusugan Program) ay nagsisilbing malawak at pinahusay na bersyon ng dating Konsulta (Konsultasyong Sulit at Tama) primary care benefit package.
• Kabilang sa mga benepisyo ang mga sumusunod:
- Free medical check-up
- Free laboratory tests
- Free medicines

👉STEP 1:
Maging miyembro muna ng PhilHealth. Pumunta sa DOH - PRPWD website at i-verify and inyong PWD ID:

Kung wala ang pangalan sa system, pumunta sa opisina ng PDAO at humingi ng updated printout ng PRPWD registration na pirmado ng head ng PDAO.

👉STEP 2:
Magtungo sa alinmang Philhealth office at isumite ang mga requirements.

👉PWD REQUIREMENTS FOR PHILHEALTH APPLICATION

( FOR 0 TO 20 YRS OLD )

IPASA ANG MGA SUMUSUNOD SA PHILHEATH OFC:
1. Duly accomplished Philhealth Member Registration Form (PMRF) ( NOTE: Kung bata ang may disability, dapat nanay o tatay ang aplikante )
2. Latest Certification of Disability or Medical Certificate of Child with Disability
3. One (1) valid ID ng aplikante, tulad ng National ID, SSS, TIN, Passport, Driver's License
4. Disability ID ng bata
5. Photocopy ng birth certificate ng aplikante mula sa PSA
6. Marriage Certificate - for married female registrant
7. In case of qualified dependent: photocopy of PSA-issued Birth Certificate

( FOR 21 TO 59 YEARS OLD )

IPASA ANG MGA SUMUSUNOD SA PHILHEALTH OFC:
1. Duly accomplished Philhealth Member Registration Form
2. Latest Certification of Disability or Medical Certificate
3. At least one (1) valid ID ng aplikante tulad ng National ID, SSS ID, TIN, Passport, Driver's License etc.
4. Photocopy ng birth certificate ng aplikante mula sa PSA
5. Marriage Certificate - for married female registrant
6. In case of qualified dependent: photocopy of PSA-issued Birth Certificate

👉STEP 3:
Kung miyembro na ng PhilHealth, pwedeng magpa-rehistro para sa YAKAP program sa mga sumusunod:
1. eGovPH App
2. PhilHealth Member Portal
3. Sa anumang PhilHealth Office sa buong bansa
4. Pinakamalapit na YAKAP clinic. Lahat ng health centers sa Taguig City ay accredited YAKAP centers

👉STEP 4A: Para sa check-ups
Pumunta sa YAKAP Clinic kung saan nakarehistro para sa magpa-check-up.

👉STEP 4B: Para sa laboratory tests
1. Pumunta sa YAKAP Clinic para sa konsultasyon. Kung kailangan ng laboratory test, magbibigay ang doktor ng referral.
2. Dalhin ang referral sa laboratory ng YAKAP clinic.
3. Kung walang laboratory sa YAKAP Clinic, maaaring magpunta sa ka-partner na diagnostic o laboratory clinic.

👉STEP 4C: Para sa libreng gamot
1. Kung may kailangang gamot, magrereseta ang doktor gamit ang GAMOT App.
2. Pumunta sa mga accredited na GAMOT provider o botika para kunin ang gamot at magdala ng government-issued valid ID
3. Ipakita sa pharmacist ang reseta o Unique Prescription Security Code (UPSC)
4. Pirmahan ang GAMOT Availment Slip na ibibigay ng pharmacist

👉MGA DAPAT TANDAAN SA LIBRENG GAMOT
• May karatulang naka-display sa labas at loob ng botika katunayang sila ay accredited na GAMOT provider
• May annual benefit limit na hanggang Php 20,000 ang halagang maaaring makuhang gamot ng kada benepisyaryo
• Sasabihin ng pharmacist kung magkano pa ang balanse mula sa Php 20,000 na limit.

👉STEP 5:
Bumalik sa YAKAP Clinic para sa mga susunod na follow-up check up. Mahalaga ang follow-up para ma-monitor ang kondisyon at epekto ng gamot. Huwag kalimutang sundin ang payo ng doktor.

4. Government Issued ID or Barangay Clearance or Utility Bill with the name and address of the applicant
5. Authorization Letter if the applicant is not available
6. School ID or Birth Certificate if applicant is still a minor
7. For Apparent Disability - Certificate of Disability (CAD) mula sa PDAO
8. For Non-Apparent Disability - Certificate of Non-Apparent Disability (CNAD) mula sa Taguig City Health Center sa inyong barangay

( Para makakuha ng CNAD, dalhin ang latest medical certificate o latest medical records issued by a private or public health specialist sa malapit na Taguig City health center for verification. For visual disability, medical certificate should be issued by an Ophthalmologist)

4. Updated Clinical Abstract or Medical Certificate
5. For Non-Apparent Disability - Certificate of Non-Apparent Disability (CNAD) mula sa Taguig City health center*
6. Proof of residence: Government Issued ID or Barangay Clearance or Utility Bill with the name and address of the applicant

( * Para makakuha ng CNAD, dalhin ang latest medical certificate o latest medical records issued by a private or public health specialist sa malapit na Taguig City health center for verification. For visual disability, medical certificate should be issued by an Ophthalmologist)
