1. at least 15-30 years old
2. Submit an Application letter addressed to MAYOR ROQUE S. VERZOSA, JR.
3. Certificate of Indigency
4. Photocopy of School card (DepEd Form 138 or secure and submit Certification of their passing General Average from their school.
