*We don't allow walk-ins as per policy
*Please prepare your Covid-19 vaccination card or Vaccination Certificate if available through a picture from phone/tablet, photocopy or card
*Please prepare your passport; photocopy or original
*Wear proper attire. No wearing of shorts during your appointment.
*The BOQ office or staff won't assist you on your booking in connection to the Anti-Fixer law.
*If you can't make it on time, please let us know within a day via BOQ Bohol's official FB messenger - Bureau of Quarantine Bohol , or from our Office number 09707754856.
*Not able to attend your appointment during the day is non-refundable and forfeitable unless you've notified the office within a day.
*We expedite clients prior to their Departure/Flight 7 days ahead or within 7 days before going out. Just bring one of these 3 proofs: International Ticket, International Itinerary, International Letter or Notice
*If the client himself/herself can't make it to the appointment, he/she can authorize his/her next of kin via Authorization letter. Special power of Attorney is needed and Authorization letter if from a friend or cousin.

*Please bring your Passport and Vaccination Card

*For Polio vaccine and certification , please come on January 3, 2022 Monday.
