Alinsunod sa PHO Memorandum No. 2026-0032 ukol sa *Rational Utilization of Hospital Resources*, Simula Hunyo 8, 2026, ipinababatid sa lahat na ang mga laboratory request, diagnostic request, at reseta mula sa RHU, pribadong klinika, at iba pang ospital ay kinakailangang maipakonsulta muna sa OPD Physician ng Bulan Medicare Hospital bago maproseso.

MGA HAKBANG SA PAGKUHA NG LABORATORY AT PHARMACY SERVICES

STEP 1️⃣: Magpakonsulta muna sa OPD Physician
-Para sa pagsusuri at pagpapatunay ng laboratory request o reseta.

STEP 2️⃣ Kumpletuhin ang mga kinakailangang dokumento
- Sundin ang mga tagubilin mula sa Laboratory o Pharmacy Staff.

STEP 3️⃣: Iproseso ang mga dokumento sa kinauukulang opisina
- Para sa kaukulang pag-apruba at beripikasyon.

STEP 4️⃣ Kunin ang serbisyo
• Magtungo sa Laboratory para sa specimen collection o pagsusumite ng sample.
• Magtungo sa Pharmacy para sa pagkuha ng gamot.

✅ Ang laboratory request ay dapat mapatunayan ng OPD Physician ng Bulan Medicare Hospital.

✅ Sundin ang mga ibibigay na tagubilin bago ang laboratory test.

Ang Bulan Medicare Hospital ay magalang na nagpapaalala sa lahat ng pasyente at bisita na laging magdala ng valid ID kapag kumukuha ng anumang serbisyo sa ospital. Ang valid ID ay kinakailangan para sa beripikasyon at upang matiyak ang maayos at mabisang transaksyon.

Para naman sa mga menor de edad na pasyente o bisita, kinakailangan nilang magsama ng legal na tagapangalaga (magulang o guardian) upang maisagawa ang mga kinakailangang proseso.

Para sa mga nais mag-avail ng serbisyo, sundin lamang po ang proseso:

1. Magdala ng isang (1) photocopy ng anumang valid ID at Laboratory Request
2. Kumuha ng charge slip sa Laboratory at;
3. Idala ang lahat ng requirements sa Billing Section ng ospital

Please be advised that our Blood Chemistry laboratory services are temporarily unavailable to undergo essential repair and maintenance.
