ON TIME REGISTRATION (WITHIN 30 DAYS FROM BIRTH) (Free/No fee)
For legitimate child (pa
rents are married)
1. marriage contract/certificate of parents
2. barangay Certification, if not born in the RHU Birthing Clinic
For illegitimate (parents not married)
1. birth certificate of parents
2. presence of parents (father to accomplish affidavit of admission of paternity and mother to accomplish affidavit to use the surname of the father)
3. barangay Certification, if not born in the RHU Birthing Clinic
4. community tax certificate (cedula)
DELAYED REGISTRATION (MORE THAN 30 DAYS FROM BIRTH)
1. All the requirements for legitimate and illegitimate children
2. At least two (2) of the following documentary evidences showing the name of the child, date and place of birth and name of parents:
a. Original or photocopy of baptismal Certificate;
b. Original or photocopy of form 137 or school records;
c. Medical records (Immunization card/hospital certificate);
d. Income tax return of registration parents/ owners of the documents
e. Voter Certification
f. SSS Form-E1
h. Insurance policy, if insured
i. Barangay certification
3. Certificate of Marriage, if married
4. PSA issued negative certification of birth
HOW TO AVAIL OF THE SERVICE
Step 1: Approach the LCRO staff
• Interview the client and verify if registered or not.
• 5 minutes
Step 2: If not registered, submit the required documents.
• Check and evaluate the required documents. (3 minutes)
• Interview the client. (5minutes)
Step 3: Secure official receipt of payment from the Municipal Treasurer Office (if applicable)
FOR LEGITIMATE CHILD:
Record the O.R. and prepare/type the Certificate of Live Birth and Notice of Posting. (8 minutes)
FOR LEGITIMATED CHILD:
Record the O.R. and prepare the Certificate of Live Birth and affidavit of legitimation (15 minutes)
FOR ILLEGITIMATE CHILD:
Record the O.R. and prepare the Certificate of Live Birth, affidavit to use the surname of the father and sworn attestation, if applicable (10 minutes)
Step 4: Read and sign the Certificate of Live Birth
• Review the COLB, subscribe the affidavits and sign (6 minutes)
• Give the claim stub to the client. (2 minutes)
Step 5: If delayed registration, return after ten (10) days of Posting to get the registered Certificate of Live Birth.
• Review and Sign the Certificate of Live Birth after ten (10) days. (5 minutes)
