🏫 Junior High School (JHS): 10:00 AM – 11:00 AM

🏫 Senior High School (SHS): 1:00 PM – 2:00 PM

1️⃣ 3 Photocopies of School ID (Front and Back) – Must clearly show your Pacita 1 address.
2️⃣ 3 Wet Signatures – Each of the 3 photocopies must have your actual, original signature signed in ink.

𝗜𝗳 𝗮 𝗚𝗨𝗔𝗥𝗗𝗜𝗔𝗡 𝘄𝗶𝗹𝗹 𝗰𝗹𝗮𝗶𝗺 𝗼𝗻 𝗯𝗲𝗵𝗮𝗹𝗳 𝗼𝗳 𝘁𝗵𝗲 𝘀𝘁𝘂𝗱𝗲𝗻𝘁:
If the scholar cannot make it personally, an authorized guardian may claim it by presenting:
1️⃣ Authorization Letter signed by the student.
2️⃣ 3 Photocopies of the Student's School ID (Front and Back) with 3 wet signatures of the student.
3️⃣ 3 Photocopies of the Guardian's Valid ID with 3 wet signatures of the guardian.
