ADVISORY
To all PNP retirees with qualified beneficiaries:
Please be informed that the following are the documentary requirements for the issuance of a Beneficiary ID.
1. FOR SPOUSE
a. Duly accomplished and notarized Retireeโs Family Information Form (RFIF), downloadable from the PRBS website at https://prbs.pnp.gov.ph;
b. Notarized Affidavit of Authenticity downloadable from the PRBS website;
c. PSA Marriage Certificate together with its corresponding Official Receipt (OR);
d. Updated PSA Advisory on Marriages (AOM) of both the retiree and the spouse, together with their corresponding Official Receipts (ORs), issued within six (6) months prior to the date of submission; and
e. Photocopy of the applicantโs National ID (PhilID) or any two national government-issued IDs bearing the applicantโs signature and thumbmark. The photocopy must be signed by the applicant and indicate the applicantโs contact number.
2. FOR CHILD
a. Duly accomplished and notarized Retireeโs Family Information Form (RFIF), downloadable from the PRBS website at https://prbs.pnp.gov.ph; (Note: To be filled out by the child, retiree, parent, or guardian)
b. Notarized Affidavit of Authenticity downloadable from the PRBS website;
c. PSA Birth Certificate of the child below eighteen (18) years of age, together with its corresponding Official Receipt (OR);
d. Photocopy of the applicantโs National ID (PhilID), School ID, or Passport bearing the applicantโs signature and thumbmark. The photocopy must be signed by the applicant/parent guardian and indicate the applicantโs/guardian contact number; and
e. For a child who is a Person with Disability (PWD), a photocopy of the Medical Certificate.
3. FOR PARENTS OF UNMARRIED RETIREE
a. Duly accomplished and notarized Retireeโs Family Information Form (RFIF), downloadable from the PRBS website at https://prbs.pnp.gov.ph;
b. Notarized Affidavit of Authenticity downloadable from the PRBS website;
c. PSA Birth Certificate of the retiree and PSA Marriage Certificate of the parents, together with their corresponding Official Receipts (ORs);
d. Updated PSA Certificate of No Marriage Record (CENOMAR) of the retiree, together with its corresponding Official Receipt (OR), issued within six (6) months prior to the date of submission; and
e. Photocopy of the applicantโs National ID (PhilID) or any two national government-issued IDs bearing the applicantโs signature and thumbmark. The photocopy must be signed by the applicant and indicate the applicantโs contact number.
