Operation Restore Hope Australia (ORHA) – Patient Sign‑Up Form Pasig City is partnering with The Mead Foundation and Operation Restore Hope Australia (ORHA) to help identify Pasigueños who may benefit from free surgery for cleft lip or cleft palate. Since 1993, ORHA and The Mead
Foundation have provided over 3,500 free reconstructive surgeries to Filipino patients. Their volunteer medical team, surgeons, anesthetists, and Filipino nurses based in the UK, USA, and Australia, offers complete, no‑cost care,including:
-Medical screening
-Laboratory tests
-Medicines
-Surgical procedures
-Post‑operative follow‑up

All services are 100% free of charge.

Who can sign up
Pasig residents (children or
adults) with:
-Cleft lip
-Cleft palate
-Previous cleft repair needing revision (subject to evaluation)

Important Dates (Tentative)
-Pre‑Screening: April 12, 2026 (Sunday)
-Mission Week: April 19–24, 2026

Location: San Juan Medical Center

What this form is for This form helps the City Health
Department identify and endorse qualified Pasigueño patients for the 2026 ORHA Medical Mission. After submitting your information, our team will contact you
regarding next steps and screening instructions.

Data Privacy
All information submitted will be kept confidential and used only for patient assessment and coordination for
this medical mission.

Operational po kami. 😘 Ngunit bago lumabas, tiyakin ang pansariling kaligtasan. Mag-ingat, maghanda, at magtiwala—nandito kami.

Alinsunod sa Memorandum Circular Blg. 106 mula sa Tanggapan ng Pangulo, mananatiling operational ang mga mahahalagang serbisyong pangkalusugan sa ating lungsod.

Kaya’t mananatiling bukas at handang maglingkod ang mga pasilidad ng City Health Department upang patuloy na makapagbigay ng kinakailangang serbisyo sa mga Pasigueño.
