2. Application Accreditation Form (available at the Sangguniang Barangay Office)
3. Duly approved Board Resolution
4. Certificate of Registration issued by the Securities and Exchange Commission (SEC) or the concerned government agencies which accredits organizations, if any.
5. List of current officers and members
6. Annual Accomplishment Report for Year 2025
7. Financial Statement for Year 2025
8. Profile indicating the purposes and objectives of our organization
9. Copy of the minutes of the CY 2025-2026 Meeting of the organization
