g. Affidavit duly executed by the applicant;
h. Marriage contract of parents, if born legitimate or born inside
the marriage;
i. Cedula of applicant/informant;
j. Any 2 documentary evidences showing the identity of the parents;
-Certificate of live birth of parents;
-Government issued ID of parents;
-Marriage contract of parents, if applicable;
-Certificate of Death for the deceased parents.
