Any of the following IDs must be presented:

âś…Postal ID
âś…PWD ID
✅Student’s ID or Library card
✅Driver’s License
âś…NBI Clearance
âś…Philippine Passport
âś…SSS ID/GSIS/UMID ID
âś…PRC ID
âś…NCIP
âś…Any other Government issued valid IDs
and must include the complete address of the place where you wish to register/transfer
The following may also be presented during registration/transfer such as PROOF OF BILLING (electricity bill, water bill, internet, credit card statements, etc. ) named after you.

đź“Ť CHANGE/CORRECTION OF ENTRIES
The following documents must be from PSA or not a Certified True Copy from the Local Civil Registrar.
âś… Birth Certificate - when there is an error in the name, date of birth, place of birth, or typographical errors
âś… Marriage Contract/Court Order - when there is a change in the name because married or validity of the court order
âś… CENOMAR - when changing status from "married" to "single"
