OFFICE OF THE CITY TREASURER – LAOAG CITY

The Office of the City Treasurer of Laoag City, Ilocos Norte hereby informs the public of the loss of accountable forms under the custody of the Revenue Collection Clerk I of the Laoag City Public Market and Commercial Complex.

The following accountable forms have been reported lost/missing:

🔴 Official Receipts – Accountable Form No. 51-C
• Original, Duplicate & Triplicate Copies
• 9 sets
• Serial Nos. 3083942 to 3083950

🔴 Cash Tickets – ₱10.00 denomination
• 1,782 pieces
• Serial Nos. 5552219 to 5554000

The accountable forms were discovered to be lost/missing on June 24, 2026, despite exhaustive efforts and due diligence to locate and recover them.

⚠️ PUBLIC ADVISORY

The public is hereby advised to exercise caution and vigilance and to guard against any fraudulent or unauthorized use of the above-mentioned accountable forms.

Any transaction involving the said lost accountable forms shall not be honored and shall be considered illegal.

Should any of the above accountable forms come into your possession, please immediately report the matter to the Office of the City Treasurer of Laoag City.
