Notarization alone does not change a person's name. While a notary public may notarize certain documents, the authority to approve a change of first name belongs to the proper government offices, and the legal process prescribed by law must still be followed.
Ubos sa Republic Act No. 9048, mahimong tugotan ang petition para sa pag-ilis sa first name o nickname kung:
(1) Ang first name o nickname sa petitioner kay kataw-anan, makapakaulaw, lisod isulat o litukon;
(2) Ang bag-ong first name o nickname dugay nang gigamit sa petitioner ug nailhan na siya sa komunidad pinaagi niini; o
(3) Ang pag-ilis sa pangalan makatabang aron malikayan ang kalibog o confusion.
To apply, a petition must be filed with the Local Civil Registrar and supported by the documentary and publication requirements prescribed by the law.
