Ang mga guidelines at requirements na amin pong ipinopost ay mula mismo sa Insurance/SMC. Ibinababa lamang po ng Barangay ang mga impormasyong ito upang maipabatid sa lahat.

Para po sa mga nagsasabing "unfair" ito dahil may mga nakatanggap na ng payout, kami po ay humihingi ng inyong pag-unawa. Sa halip na magreklamo, mas makabubuti po na sabay-sabay nating kumpletuhin ang mga kinakailangang dokumento.

Hindi po layunin ng Barangay na pahirapan ang sinuman. Nais po naming maclaim ng lahat ang tulong na ito sa tamang proseso at oras.

📢 PAALALA: MGA KAILANGAN PARA SA PAYOUT – PRAYORIDAD NA BENEPISYARYO

Ipinapaabot po namin sa lahat ng concerned na indibidwal na ang payout para sa pinansyal na tulong ay isasagawa ayon sa priority system batay sa naberipikang katayuan ng pamilya at kompletong dokumentasyon. Mangyaring basahin at sundin ang mga sumusunod na requirements upang maging maayos ang proseso ng inyong claim:

1. Mag-asawa (Married Couples)
Mga Kailangang Dokumento:
✔️Valid Government ID ng claimant na may kumpletong address at Purok
(Kinakailangan ang Voter’s Certificate kung hindi kumpleto ang address)
✔️Valid ID ng asawa (kung mayroon)
(Isulat ang N/A kung hiwalay na)
✔️ID ng anak (e.g., school ID)
(Isulat ang N/A kung walang anak)
✔️Marriage Certificate (photocopy o Barangay Certificate kung wala ang original)
✔️Birth Certificate ng mga anak (kung mayroon)

2. Awtorisadong Tagakuha (Authorized Claimant – 1st Family Only)
Mga Kailangang Dokumento:
✔️Valid Government ID ng benepisyaryo na may address at Purok
(Kinakailangan ang Voter’s Certificate kung hindi kumpleto ang address)
✔️Valid ID ng partner ng benepisyaryo (kung mayroon)

4. Mga Kapatid (Siblings)
Mga Kailangang Dokumento:
✔️Valid ID ng claimant na may address at Purok
(Kinakailangan ang Voter’s Certificate kung hindi kumpleto ang address)
✔️Birth Certificates ng mga kapatid
✔️Death Certificate ng parehong yumaong magulang

Mahalagang Paalala:
Ang mga special cases gaya ng:
👉🏼❗Mga live-in partners (mayroon o walang anak)
👉🏼❗Mga hindi kinikilalang pinuno ng pamilya
👉🏼❗Mga miyembro ng LGBTQ+
👉🏼❗Dalawang familyhead ng pamilya, at iba pang di-karaniwang sitwasyon

We are informing all concerned individuals that the payout for financial assistance will follow a priority system based on verified family status and documentation. Please refer to the requirements listed below to ensure smooth processing of your claims.

1. Married Couples
Required Documents:

✓Claimant’s Government-issued Valid ID with complete address and Purok(Voter’s Certificate required if address is incomplete)
✓Partner’s Valid ID with complete address (if available)
(Indicate N/A if separated)
✓Children’s ID (e.g., school ID) (Indicate N/A if no children)
✓Marriage Certificate (Photocopy required)
(Barangay Certificate of Marriage acceptable if original is lost)
✓Children’s Birth Certificate (if applicable)
✓Death Certificate of deceased partner (if applicable)

2. Authorized Claimant (1st Family Only)
Required Documents:

✓Beneficiary’s Government-issued Valid ID with complete address and Purok(Voter’s Certificate required if address is incomplete)
✓Beneficiary’s Partner’s Valid ID (if available)
(Indicate N/A if separated)
✓Death Certificate of deceased partner (if applicable)
✓Marriage Certificate
✓Authorized Claimant’s Government-issued Valid ID
✓Authorization Letter
✓Birth Certificate of the Authorized Claimant showing relationship to the beneficiary
✓Children’s ID (e.g., school ID) (if applicable)
✓Children’s Birth Certificate (if applicable)

3. Senior Citizens
✓Follow the same requirements as for Married Couples.
✓Still eligible even if children have their own families
✓Death Certificate of deceased partner (if applicable)

4.Siblings
Required Documents:

✓Claimant’s Government-issued Valid ID with complete address and Purok
(Voter’s Certificate required if address is incomplete)
✓Siblings Birth Certificates
✓Death Certificate of deceased parents

Important Note:

Special cases involving non-standard family arrangements—such as live-in partners (with or without children), non-family heads, members of the LGBTQ+ community, double family heads, and other unique circumstances—will only be processed AFTER the payout to compliant and eligible primary beneficiaries has been completed.

We encourage everyone to prepare the required documents ahead of time to avoid delays in disbursement.

Please bear with us as we await further updates on the complete list of requirements. We are encouraging everyone to start preparing the following originals and photocopies:

👉 Claimant's ID (must have a complete address; if the address is incomplete, please secure a Voter’s Certification. If the purok listed is still different, you will need to obtain a Barangay Certification confirming your purok transfer.)
👉 Birth certificates of the children (regardless of the address)
👉 ID of the Children (regardless of the address)
👉 Marriage certificate
(Regardless of the address)
👉 Spouse’s ID
(Regardless of the address)
👉 Death certificate (for widows/widowers) (regardless of the address)

These are among the documents that will likely be required, so please have them ready in advance.
