Day 1 of the submission of SK Polo Dacu Educational Assistance Application Form.

Moreover, SK Polo Dacu reminded all that only Senior High School (Grade 11 and Grade 12) as well as College students are allowed to apply for the said assistance.

Furthermore, only one per family is allowed because of limited slot available which composed of 25 SHS and 25 College students.

WHAT: "SK EDUCATIONAL ASSISTANCE PROGRAM 2024 OF BARANGAY POLO DACU"

Calling the attention of all Senior high school students and college students of Brgy. Polo Dacu.We would like to inform you that we will be having an educational assistance.

This assistance aims to help alleviate any financial burdens you may be facing and to ensure that you can continue your valuable contributions to our community.

WHO CAN APPLY
* SHS students(25 slot's ) residing in Brgy. Polo Dacu
* College Students (25 slot's) residing in Brgy. Polo Dacu

QUALIFICATIONS
*Must be a register voter and resident of barangay Polo Dacu.
* No dropped subject/ failed
*15- 30 years old
*Willing to participate on program and activities conducted by the SK official's

To avail the Educational assistance kindly bring the following requirements.

SENIOR HIGH SCHOOL
* 2 copies of School ID or any valid ID
* Brgy. Indigency Certificate
*Prospectus/ report card (photocopy)
* SK POLO DACU educational assistance form( to be given by SK POLO DACU official's)

COLLEGE
* 2 copies of validated School I.d or any valid I.d( with certified through copy and original signature validated by the school registrar
*Certificate of Enrollment
* SK POLO DACU educational assistance form( to be given by SK POLO DACU official's)

REMINDERS
* Only 1 per family is allowed to apply.
* Only 25 Senior High School students and 25 College Students will be accomodate for this Educational Assistance.
* Late submission will not be Accomodated.
* Final list of students for Educational Assistance will be posted on this page.
* Failure to complete the requirements will not be entertained.

DEADLINE OF SUBMISSION
* July 1 - July 7
8:00 - 11:00 a.m
* Pass the requirements at Brgy. hall of Polo Dacu.
