Attention, Mangkasanons! Registration for the PhilSys National ID will be available at the Serbisyo Caravan starting tomorrow at Barangay 1. If you haven't registered yet or need to complete your PhilSys step, securing your official identification is now easier than ever.

To register, you only need to bring ANY ONE (1) of the following options:
1. Original Birth Certificate
2. Original Barangay Certificate (Note: This must include your complete personal details and an attached 2×2 or 1×1 photo)
3. Any Valid ID

Lacking basic identity documents or need to fix a delayed birth registration? The Office of the Local Civil Registrar is coming directly to you at the Serbisyo Caravan starting tomorrow at Barangay 1!

Save yourself a trip to City Hall and check the requirements below to get your documents sorted out right in your neighborhood:

👶 1. APPLICATION FOR LATE REGISTRATION OF BIRTH
If you or a family member's birth was never officially registered, please prepare the following:
• Negative Result from PSA
• Certificate of Live Birth (Hospital/Lying-in)
• Marriage Contract of Parents
• Marriage Contract of Applicant
• Community Tax Certificate
• National ID
• Barangay Certification
• 1 piece 2x2 white background picture
• PLUS: Bring ANY TWO (2) of these supporting documents showing your date and place of birth: Baptismal Certificate, Growth Chart, Senior Citizen Application Form/ID, PhilHealth MDR, Service Record, GSIS Record, SSS Form E-1/E-4, or School Record.

👩‍👦 2. LATE REGISTRATION OF BIRTH FOR ILLEGITIMATE CHILD
For processing late registrations under this specific category, the following legal documents are required:
• Sworn Statement of Mother
• Affidavit of Guardianship
• Joint Affidavit of 2 witnesses
• Affidavit to Use the Surname of the Father (AUSF)

Need to request official records from the national database? The caravan will offer direct filing assistance for requesting the following civil registry documents from the PSA:

The Office of the Persons with Disability Affairs (OPDA) will be part of the Serbisyo Caravan starting tomorrow at Barangay 1 to help you or your family members apply for a PWD ID with ease.

If you are a resident of La Carlota City looking to avail of the ID and its accompanying benefits, please make sure to prepare these 4 core requirements:

1.) Application Form – Secured and filled out from the OPDA Office (available on-site during the caravan!).
2.) Barangay Certificate of Residency – Certifying that the applicant is a resident of La Carlota City.
3.) Medical Certificate or Certification – Duly signed by a City Health Office Physician (Medical Doctor).
4.) 3 pieces of 1x1 ID Pictures – Must be recent.

To our beloved elder Mangkasanons, getting your official Senior Citizens ID is now closer and easier than ever! The Office of the Senior Citizens Affairs (OSCA) is participating in the Serbisyo Caravan starting tomorrow at Barangay 1 to process applications right in your community.

To avail of your ID and access your statutory discounts and benefits, please bring the Standard Requirements:

1.) Birth Certificate
2.) 2 pieces of 1x1 ID Picture
3.) Valid ID showing your Residence Address (For married women, 1 valid ID is enough)

No Birth Certificate available? Don't worry! You can bring ANY of these alternatives instead:
* Baptismal Certificate
* Joint Affidavit of Birth Certificate
* Any of the following valid IDs: COMELEC ID, SSS ID, GSIS UMID, PhilHealth ID, Driver’s License, or National ID
