- Photocopy of a Valid ID or School ID with Bayan Luma 3 Address
- MINORS WITHOUT ANY VALID ID MUST PRESENT THEIR PARENT'S/GUARDIAN'S PHOTOCOPY OF ANY VALID ID
- Fully paid quota

Reminders:

- INCOMPLETE REQUIREMENTS, NO PLAY
- NO QUOTA = DISQUALIFICATION OF TEAM SLOT
- NO JERSEY, NO PLAY
- Failure to submit the team’s lineup and requirements would result in forfeiture of the slot
