ANNOUNCEMENT:
PSA-CRS MOBILE OUTLET-on site issuance
Ang Pamahalaang Bayan ng Ibaan, Batangas ay magsasagawa ng PSA-CRS MOBILE OUTLET-on site issuance sa inisyatibo ni HON. MAYOR JANE A. CASAS at suporta ng Sangguniang Bayan ng Ibaan sa pangunguna ni Hon. Vice Mayor Edralyn Joy A. Salvame, sa pakikipagtulungan ng Office of the Municipal Civil Registrar at Philippine Statistics Authority. Layunin ng naturang aktibidad na mailapit sa mga mamamayan ng Ibaan ang serbisyo publiko na may kaugnayan sa pag request ng PSA Copy ng Civil Registry Documents tulad ng CENOMAR, Birth, Marriage, Death Certificates, at iba pang dokumento na karaniwang hinihingi sa mga pakikipag transaksyon sa ibat-ibang tanggapan maging pambubliko man o pampribado.
Petsa: April 21, 2026 (Tuesday) 8:00am-5:00pm
Lugar: Recto Gymnasium, Poblacion, Ibaan, Batangas
Paalala: First come first serve basis (Maximum of 120 requesters only for PSA Civil Registry Documents requests)
Requirements:
If the requester is the:
• Owner of Document: 1 government-issued ID
• Spouse of Document Owner: Certificate of Marriage and 1 government-issued ID
• Parent of Document Owner: Certificate of Live Birth of Child and 1 government-issued ID
• Direct Descendant of Document Owner: Certificate of Live Birth of Direct Descendant and 1 government-issued ID
• Requesting for Persons not specified above: Authorization letter (AL)/Special Power of Attorney (SPA) indicating the details of the document to be requested with purpose and number of copies needed, date of AL & SPA, name of Office concerned, original/photocopy of valid ID of the document owner and original valid ID of the authorized requester/Attorney-In-Fact
