📍 All students from Cauayan are scheduled at the Cauayan Primary Health Facilities on May 6–7, 2026, from 1:00 PM to 4:00 PM only.

📍 All students from Ilog are scheduled at the Ilog Primary Health Facilities (Rural Health Unit) on May 7, 2026, at exactly 3:00 PM only.

📄 Kindly bring your laboratory results, medical health record, and medical certificate for signing.
