4. Photocopy of Authenticated Certificate of CS Eligibility/Board Rating and Licenses for RA 1080
5. Photocopy of Certificate of Employment
6. Photocopy of Relevant Trainings/Seminars attended (if any)
7. Photocopy of Voter’s I.D/Certificate
8. Photocopy of NBI Clearance
9. Photocopy of Birth Certificate and Marriage Certificate (if applicable)
10. Performance Rating (Appraisal) or Individual Performance Commitment Review in the last rating period (if any)
