Lists of Registrants for Verification under PhilSys Birth Registration Assistance Project (PBRAP).
Adunay umaabot nga Registration of Birth sa mga Late Registrants karong October 28 & 29, 2024 sa Brgy. Hall, Katangawan kauban ang Local Civil Registrar (LCR) Office.
Ang mga naa sa listahan nga NEGATIVE RESULT/WALAY P.S.A RECORD pwede mo mag process sa inyong mga Birth Certificates.
Ang lists of REQUIREMENTS makuha diri sa Barangay Hall.
Mao kini ang mga sumusunod na dal-on during sa Registration day:
1. Marriage Contract sa (Parents/Registrants)
2. Cedula year 2024/Valid ID sa (Mother/Father/Registrant)
3. At least duha (2) lang ka documentary evidences nga ga justify sa imong pangalan, birthdate, birth place ug pangalan sa parents of the registrants sama sa:
📌Baptismal/dedication certificate
📌School records (form 137/Transcript/
Pupils profile enrollment record/school certification
📌Voter's registration application/Voter's Registration Record
(makuha kini sa COMELEC)
📌GSIS records/SSS record
📌MDR (Philhealth)
📌MDF ( Pag-ibig)
📌Passport
📌Service Record/Employees Profile
📌Birth/Baptismal Certification of Siblings/Children
📌National ID
For 0-6 years old:
1. Immunization card
2. New-born screening
3. Brgy. Certification of birth
4. Pre- natal record/Ultrasound result
5. SSS/GSIS maternity claim
