UNSAY DALAHON?

📣KUNG AND PWD ANG MOKUHA:

1. Certificate of eligibility (makuha sa brgy. hall)
2. Barangay Certificate (confirmed as a registered voter)
3. Photocopy sa PWD ID (back to back)

📣AUTHORIZE REPRESENTATIVE:

1. Pagtugot sa pagdawat/Authorization Letter CSWDO format (makuha sa brgy.hall)
2.Barangay certificate confirmed as a registered voter. (Pangalan sa PWD nga cert. Ang naka indicate)
2. Photocopy sa PWD ID nga pirmado ikatulo.
3. Photocopy ID sa mag representative nga naka pirmado katulo.
4. Dalahon ang original PWD ID ug sa mag representative.

Palihog sa pagbasa ug tarong aron masabtan ang mga instructions.
