2. If there are discrepancies in the names and address
a. Brgy. Cert of One and the Same
b. Brgy Cert of Residency

3. Only immediate family and living in the same house can claim as representative
a. Authorization
b. Govt Valid ID of the Representative and the Original Representative with signature

4. If the common law partner will claim as representative
a. Brgy Cert of as Common Law Partner
