Handler's License and Transport Carrier Registration minimum standards and requirements will also be discussed.

Certificate of Attendance from any Animal Welfare Seminar/Webinar does not expire, those who attended previously need not attend.

Requirements for the movement of GAME FOWLS

1. BAI Certificate of Poultry Farm Accreditation (1 photocopy)
WHERE TO SECURE? BAI - Animal Health and Welfare Division

2. Veterinary Health Certificate with Newcastle Disease (ND) vaccination, ND and Avian Influenza free Certification (1 original)
WHERE TO SECURE? If issued by farm veterinarian, it must be concurred by LGU(Municipal/City/Provincial) or DA Regional Field Office veterinarian

3. Laboratory Compliance Report for Avian Influenza (1 original)
WHERE TO SECURE? BAI - Animal Health and Welfare Division

4. Livestock Handlers License (1 photocopy)
WHERE TO SECURE? BAI, DA Regional Field Office, LGU Office of the Provincial Veterinarian

For farms without BAI Certificate of Poultry Farm Accreditation:

1. Negative Avian Influenza laboratory test result shall be required prior to shipment (1original)

2. Veterinary Health Certificate

Requirements for the Application of LOCAL SHIPPING PERMIT REQUIREMENTS FOR
SWINE (For Slaughter)
1. Livestock Handler’s License
2. Registration of Transport Carrier
3. VHC indicates NO reported cases/outbreaks of priority diseases in the source.
4. Certificate of Free Status on African Swine Fever (CFS-ASF) - (For Commercial Farm)
5. Recognition on Active Surveillance on African Swine Fever (RAS-ASF) - (For Backyard Farm)
6. Disinfection Clearance form the Border of Entry (GATAS, KALAWIT, ZAMBOANGA DEL NORTE )

Requirement for LOCAL SHIPPING PERMIT REQUIREMENTS FOR DOGS AND CATS:
✔ Veterinary Health Certificate (VHC) issued by a Licensed Veterinarian.
⮚ Valid for 3 days from the date of issuance.
The VHC shall include the following Rabies Vaccination details:
● Vaccine brand name, batch and lot number
● Date of administration and date of next vaccination.
● The VHC shall be accompanied by a rabies vaccination certificate bearing the signature over full printed name of the Licensed Veterinarian, PRC number, and valid PTR number (for private practitioners).
✔ AGE: 3 ½ -4 months and above shall be allowed for shipment.
The animal shall be vaccinated against rabies not more than one (1) year but not less than fourteen (14) days at the time of shipment. For annual booster shots, the dogs/cats may be shipped immediately upon vaccination.
