REQUIREMENTS FOR NEW BUSINESS PERMIT

1. SEC/CDA- for Corporation, Cooperative, Partnership, etc. (1 photocopy)
2. DTI- for Single Proprietorship (1 photocopy)
3. Certificate of Occupancy (If applicable)
Building Permit for Light Material Structures not more than 20
sq.m.
4. Barangay Business Clearance (1 photocopy)
5. Cedula
6. BIR Payment

REQUIREMENTS FOR RENEWAL OF BUSINESS PERMIT
1. Old Business Permit
2. Affidavit- Declaration of Previous Year's Gross Income
3. SEC/CDA- for Corporation, Cooperative, Partnership, etc. (1 photocopy)
4. DTI- for Single Proprietorship (1 photocopy)
5. Certificate of Occupancy (If applicable)
