✅Original Birth Certificate of the Child
✅National ID sa kauban nga ginikanan
👉For Registration of 1-4 years old kauban ang guardian;
✅Original Birth Certificate of the Child
✅National ID (PhilID/ePhilID) sa guardian nga kauban
✅Authorization letter (provided by Philsys)
✅1 Valid ID of the parent who signed the authorization letter
👉For Registration of 5 years old and above
✅1 VALID ID, or
✅Original Birth Certificate, and/or
✅Marriage Contract (if applicable)
