4S Kontra Dengue: Every Week, Every Barangay, Everyone.

The Municipality of Carles, through the Office of the Municipal Mayor, is strengthening its dengue prevention efforts through Memorandum No. 09A-2026, mandating all 33 barangays and LGU offices to conduct a weekly 4S Kontra Dengue Clean-Up Activity.

-SEARCH AND DESTROY – Eliminate mosquito breeding sites
-SELF-PROTECTION – Protect against mosquito bites
-SEEK EARLY CONSULTATION – Act early when symptoms occur
-SAY YES TO FOGGING WHEN WARRANTED – Support authorized fogging activities when needed

Weekly Schedule

📍 Barangays: Conduct the clean-up at least once a week, with Thursday at 4:00 PM as the recommended schedule. Barangays may adjust their schedule based on local circumstances.

📍 LGU Employees/Offices: Every Thursday at 4:00 PM, all LGU offices and employees shall practice the 4S within their respective workplaces and surrounding areas.

Barangays are required to submit weekly photo documentation and accomplishment reports to MENRO using the prescribed template provided by MENRO.

Maaari niyo bang papalitan ang apelyido ng anak ninyo at gamitin na ang apelyido ng tatay nito, kahit namayapa na ito? 🤔

Puwede parin po, at may dalawang paraan upang magawa ito: 🤔

1. Kung acknowledged naman po ng ama ang bata noong buhay pa ito sa pamamagitan ng: 🤔

a. Pagpirma bilang ama sa birth certificate nito;
b. O sa pamamagitan ng isang Affidavit of Acknowledgement na dokumento;
c. O sa pamamagitan ng isang sulat kamay nitong liham tungkol sa bagay na ito; 🤔

Then sa isang administrative proceeding lang po under RA 9255 ang pagdadaanan ninyo. 🤔

2. Subalit kung hindi po pormal na inacknowledge ng ama ang anak ninyo habang buhay pa ito; 🤔

Then isang judicial proceeding na po ang pagdadaanan ninyo at sa korte na po kayo tutungo. 🤔

Sapagka't kailangan niyo na pong kumbinsihin ang husgado tungkol sa filiation ng anak ninyo. 🤔

At mga Ka-G ko, kung umabot po hanggang dito ang pagbabasa ninyo, at nais niyong magkaroon rin ng mahiwagang maaaggs ko; 🤔

WALANG BIRTH CERTIFICATE

Alam mo ba na maaari pa ring iparehistro ang isang kapanganakan kahit lumampas na sa itinakdang panahon para sa regular na registration?

Ang Delayed Registration of Birth ay pagpaparehistro ng kapanganakan pagkalipas ng 30 araw mula sa petsa ng kapanganakan.

Mahalaga ang birth registration dahil ang birth record ay nagsisilbing mahalagang dokumento ng iyong pagkakakilanlan at kapanganakan at maaaring kailanganin sa pagkuha ng iba't ibang dokumento, serbisyo, at benepisyo ng pamahalaan.

📌 𝗦𝗮𝗮𝗻 𝗺𝗮𝗴𝗽𝗮𝗽𝗮𝗿𝗲𝗵𝗶𝘀𝘁𝗿𝗼?

Sa Local Civil Registry Office (LCRO) ng lungsod o munisipalidad kung saan naganap ang kapanganakan.

📄 May mga kinakailangang dokumento at proseso na dapat sundin depende sa sitwasyon ng ipaparehistro.

👉 Huwag hayaang manatiling walang birth record at walang Birth Certificate. Kung ikaw o isang kapamilya ay hindi pa rehistrado, makipag-ugnayan sa inyong LCRO upang malaman ang mga kinakailangan para sa delayed registration.

Irehistro ang kapanganakan. I-secure ang pagkakakilanlan.
