👉Photocopy of a valid ID with three (3) specimen signatures

For Authorized Representatives (immediate family members only):
👉Photocopy of the claimant’s valid ID with three (3) specimen signatures
👉Photocopy of the authorized representative’s valid ID with three (3) specimen signatures
👉Authorization letter from the claimant stating:
-Appointment of the representative
-Reason for inability to personally claim the assistance

We are pleased to inform you of the SCHOLARSHIP PAYOUT for the 2nd Semester, SY 2025–2026.

📌 Please be guided by the following schedule:

🗓️ May 19, 2026
📍 Venue: Fuerte Sports Complex, Cadlan, Pili
🕕 Time: Strictly at 7:00 AM
(We aim to finish early, so please arrive on time.)

✅ WHAT TO BRING:

1. School ID or any valid government-issued ID
2. If the student is unavailable, an immediate family member (Mama, Papa, or Tugang) may claim the payout on their behalf. Please bring the following:
Authorization letter from the student
1 photocopy of the student’s ID
1 photocopy of the parent’s ID
3. Please take note of your assigned number and payroll number.

⚠️ Important Reminder:
This is the only schedule for the payout. Attendance is required, and no other release schedule will be accommodated.
