Ang Philippine Coconut Authority – Camarines Norte Provincial Office ay inaanyayahan ang lahat ng coconut farmers ng Barangay MANTUGAWE para sa:

1. VALID GOVERNMENT ID (xerox copy, front at likod)
Halimbawa:
National ID, SSS ID, UMID, Postal ID, TIN ID, Passport, PRC ID, OWWA ID, DOLE Card, Voter's ID, Senior Citizen's ID, Driver's License, Barangay ID, School ID
(Kung walang alinman sa mga ID na nabanggit, tanggap ang Certificate of Residence na pinirmahan ng Barangay Chairman.)
2. PROOF OF OWNERSHIP, TENANTSHIP, o HIRED LABOR (xerox copy)
Para sa Farm Owners:
- Certificate of Land Transfer (TCT)
- Emancipation Patent (EP)
- Individual Certificate of Land Ownership Award (CLOA)
- Agricultural Sales Patent
- Homestead Patent
- Free Patent
- Certificate of Title or Regular Title
- Certificate of Ancestral Domain Title
- Certificate of Ancestral Land Title
- Tax Declaration
- Extrajudicial Partition Partition
- Deed of Sale

Para sa Farm Tenants:
- Lease Holding Agreement o sertipiko ng kasunduan sa Farm Owner
- Kopya ng Valid ID ng Farm Owner
Para sa Farm Workers/Laborers:
- Sertipiko ng kasunduan sa Farm Owner/Tenant
- Kopya ng Valid ID ng Farm Owner
3. Latest Picture ng Farmer (2x2 inch, aktwal na kuha)
