• Photocopy of latest Grades (Screenshot accepted)
• Certificate of Indigence or original Certificate of Low Income issued by Barangay or DSWD/CSWD, including both parents’ occupations
• Photocopy of latest Income Tax Return (ITR) of parents/legal guardian or BIR certification of tax exemption
•2 pcs. 2x2 ID Picture
