-Kindly disregard this notice if the senior citizen included in the list has passed away or is currently receiving any pension.
-In case of spouses, only one spouse may claim.
-Authorized representatives may claim for seniors who are unavailable due to medical reasons.

Authorized representatives must present the following upon claiming:
1. Letter of Authorization
2. Senior Citizen ID of the beneficiary
3. Valid ID of the authorized representative
