Active Case Finding (ACF) Initiative Boosts Tuberculosis Screening in Gurez Valley, Kashmir.

Gurez Valley, Kashmir - Oct,6 2023 - In an ambitious effort to enhance Tuberculosis (TB) screening and awareness in the picturesque Gurez Valley of Kashmir, a comprehensive Active Case Finding (ACF) initiative was undertaken by NTEP - Baramulla from 03-10-2023 to 05-10-2023. The campaign, supported by State TB Cell - Directorate of Health Services, Kashmir, successfully reached out to the community and engaged various partners to strengthen the fight against TB.

Key Highlights of the ACF Initiative:

*1. Mobile Molecular Testing with CBNAAT Van:* A state-of-the-art *CBNAAT (Cartridge-Based Nucleic Acid Amplification Test) Van* was deployed in Gurez Valley to facilitate rapid and accurate Tuberculosis diagnosis. This mobile unit played a vital role in conducting molecular tests for TB in remote areas of Gurez Valley including Bagtore, Tulail, Sheikhpura, Baduab and Gujran. The additional Quadro-TruNaat Machine was also installed in the same Van was made available for robust approach to screen more population.

*2. Ultra Digital Portable Handheld X-Ray Machine:* Molbio India Limited generously provided a portable handheld X-Ray Machine on the request of the State TB Officer - Kashmir to facilitate chest X-rays for individuals exhibiting TB symptoms. This advanced technology enabled on-site radiographic examinations, expediting the identification of potential TB cases and providing real-time Report of the same with embedded AI Technology.

*3. Community Engagement:* The State TB Officer, along with the District Tuberculosis Officer Baramulla and Team, visited the Bagtore and Tulail areas of Gurez Valley. During their visit, conducted PRI (Panchayati Raj Institutions) meetings and community meetings to raise awareness about TB and promote the *"TB Free Panchayat"* initiative.

*4. Extensive Screening Efforts:* A total of 1,490 individuals were screened for TB during the ACF campaign from 03-10-2023 to 05-10-2023. Among them, 216 sputum samples were collected for sputum examination. Of these samples, 89 were tested using CBNAAT and TruNaat technology, while 127 were subjected to microscopy for TB detection.

*5. Collaboration with Defence:* The ACF team collaborated with Local Units of Defence, including the Indian Army, Border Security Force (BSF), and Jammu and Kashmir Police, to raise awareness about TB. This partnership was instrumental in extending the reach of TB education and screening efforts.

This ACF initiative in Gurez Valley stands as a testament to the commitment of the National TB Elimination Programme, and Health Services, Kashmir in addressing the TB challenge in challenging terrains. The comprehensive approach of combining technology, community engagement, and collaboration with PRIs and Defense Forces has proven to be effective in reaching and screening a significant number of individuals for TB.

[Sopore, 05-09-2023] – Nikshay Diwas, a monthly event dedicated to raising awareness about Tuberculosis (TB) and its prevention, marked another significant milestone by focusing on the theme "Women and TB." This month's celebration aimed to shed light on the unique challenges that women face in the fight against TB and the importance of gender-sensitive approaches to TB care and prevention.

Tuberculosis remains a global health concern, and it affects millions of people worldwide. However, it's crucial to recognize that TB affects women differently from men due to various social, economic, and biological factors. The theme "Women and TB" seeked to address these disparities and strengthen the commitment to women's health and well-being.

Key Highlights of Nikshay Diwas September 2023 were:

1. Gender Disparities: Women are more vulnerable to TB due to factors like lower socioeconomic status, limited access to healthcare, and higher caregiving responsibilities. Nikshay Diwas emphasized the need to address these disparities.

2. Empowering Women: The event showcased stories of resilient women who have overcome TB, highlighting the importance of early diagnosis and treatment.

3. Access to Healthcare: Community Health Officers (CHOs/MLHPs) under the guidance of Sr. Treatment Supervisor - NTEP discussed strategies to improve women's access to TB diagnosis, treatment, and support services.

4. Awareness Campaigns: Nikshay Diwas launched awareness campaigns targeting women and communities to increase knowledge about TB prevention, symptoms, and available resources.

Nikshay Diwas - September 2023 encouraged everyone to join in the fight against TB and support women's health. Together, we can work towards a world where TB no longer poses a threat to the well-being of women and their communities.

District TB Officer - Baramulla Visits Ayushman Bharat - Health and Wellness Centres, Reinforces Commitment to TB Elimination in Baramulla.

May 30, 2023: The District TB Officer (DTO) today conducted a comprehensive visit to the Ayushman Bharat Health and Wellness Centres (AB-HWCs) located across the district. The purpose of the visit was to assess the implementation of tuberculosis (TB) elimination measures and strengthen the collaboration between National TB Elimination Programme and primary healthcare services.
Recognizing the critical role of these centers in achieving the TB Elimination goal, the DTO made this visit to ensure that TB control and elimination strategies are effectively integrated into the existing healthcare framework.

During the visit, the DTO interacted with MLHPs/CHOs, ANMs, ASHAs and other stakeholders involved in TB control. The discussions focused on strengthening the identification, diagnosis, and treatment of TB cases at the grassroots level. The DTO emphasized the importance of early detection, prompt treatment initiation, and adherence to the standardized treatment regimen to prevent the transmission and progression of TB in the community.

In addition, the DTO conducted a comprehensive review of the facilities and resources available at the AB-HWCs. This assessment aimed to identify any gaps or areas requiring improvement to enhance the quality and accessibility of TB services. The DTO expressed appreciation for the efforts made by the MLHPs/CHOs in delivering high-quality care and acknowledged the challenges they face in tackling TB in the district. The DTO instructed the Supervisory Staff to make every resources available to the facilities in order to achieve TB Free Panchayat Goal.

The visit concluded with the DTO outlining a set of recommendations to further strengthen TB elimination efforts in collaboration with the AB-HWCs. These recommendations include enhancing training and capacity-building programs for healthcare providers, improving diagnostic services, ensuring regular supply of anti-TB medications, and strengthening the monitoring and evaluation mechanisms.

The collaboration between the National TB Elimination Programme and the Ayushman Bharat Health and Wellness Centres will undoubtedly contribute to improving the overall health outcomes and achieving the goal of TB Free Panchayats in the district.

District TB Officer Baramulla/Bandipora visited Chief Medical Officer Bandipora, reviewed the progress of NTEP and provided Hands on Training on Tuberculosis to MLHPs/CHOs during Visit.

Bandipora: In a collaborative effort to combat the menace of tuberculosis (TB) and strengthen the healthcare system, the District TB Officer today visited the Chief Medical Officer's office and discussed the progress of the National TB Elimination Programme in Bandipora District.

Dr. Hafizullah - DTO Baramulla also conducted a hands on training session for MLHPs under the National Health Mission (NHM). The training aimed to enhance the MLHPs' knowledge and skills in the collection of samples, transportation, diagnosis, treatment, and prevention of TB.

During the visit, the District TB Officer emphasized the importance of early detection and timely intervention in tackling TB effectively. The training session provided MLHPs with up-to-date information on the latest advancements in TB management, including diagnostic techniques, treatment regimens, and strategies for ensuring patient adherence to medication.

Acknowledging the pivotal role played by MLHPs in the delivery of primary healthcare services, the Chief Medical Officer emphasized the need for continuous training to stay updated on the evolving field of TB management.

The District TB Officer encouraged MLHPs to actively engage in community outreach programs, organize awareness campaigns, and facilitate the identification and referral of individuals suspected of having TB. Additionally, the importance of maintaining accurate records and reporting cases promptly to facilitate effective monitoring and evaluation was emphasized.

The District TB Officer extended gratitude to the Chief Medical Officer, the NHM, and all MLHPs for their active participation and commitment to fighting TB. It is through such collaborative efforts that the district aims to reduce the burden of TB and improve the overall health and well-being of its residents.

Also Under the Banner of TB Harega Desh Jeetega, the Supervisory Staff of the district continued their visits to AB-HWCs, NTPHCs and PHCs across district Baramulla and Bandipora with the aim to screen maximum population across district for early diagnosis and treatment.

Review Meeting held at District TB Centre Baramulla under the chairmanship of District Tuberculosis Officer Baramulla.

Agenda:

Meeting Summary:

1. Review of TB Treatment and Case Management:
- The meeting began with a comprehensive review of TB treatment and case management in the district. The team discussed the number of new cases diagnosed, ongoing treatment progress, and adherence to the treatment regimen from January 2023.
- Sr. Treatment Supervisors shared their experiences and challenges in delivering effective treatment, including managing Adverse Drug Reactions (ADRs), ensuring medication availability, Timely DBT and addressing patient concerns.
- Strategies were discussed to enhance patient support services, including counseling, nutritional support, and active follow-up to improve treatment outcomes.

2. TB Surveillance and Reporting:
- Mr. Mir Mansoor - STS HQs presented an overview of the TB surveillance system, including data collection, reporting, and analysis for Sub National Certification (SNC) 2023.
- The discussion focused on the accuracy and completeness of data, identifying any gaps or discrepancies in reporting, and exploring ways to strengthen the surveillance system with the aim to achieve TB Free status of the District.
- It was emphasized that timely and accurate reporting is crucial for monitoring the TB situation in the district, identifying areas of concern, and planning appropriate interventions.

3. Laboratory Services and Diagnostics:
- The Programme Management Unit - NTEP Baramulla provided an update on the availability and functionality of TB diagnostic facilities in the district.
- Challenges related to the timely availability of diagnostic tests, maintenance of equipment, and training of laboratory staff were discussed.
- Strategies were proposed to improve the efficiency of laboratory services, including strengthening the supply chain, conducting regular equipment maintenance, and organizing training sessions for laboratory personnel and strong referral system across all PHIs and Designated Microscopic Centres

4. Training and Capacity Building:
- The meeting discussed the importance of training and capacity building for General Health Staff and Ashas involved in NTEP.
- Plans were made to conduct training workshops, both for laboratory staff, MLHPs/CHOs and BPMEOs, to update their knowledge and skills and ensure quality TB care delivery.

5. Patient Education and Awareness Campaigns:
- The team reviewed the ongoing patient education and awareness campaigns in the district.
- It was noted that while efforts were being made to raise awareness about TB and its prevention, more targeted campaigns were needed, especially in high-risk populations.
- Strategies were discussed to engage community leaders, NGOs, and other stakeholders to increase awareness and reduce the stigma associated with TB.

6. Challenges and Concerns:
- The meeting provided an opportunity for participants to share their challenges and concerns related to TB control in the district.
- Issues such as limited resources, staff shortages, and the need for better coordination between General Health Staff and NTEP were highlighted.
- The Administration committed to addressing these concerns and exploring feasible solutions to improve the overall performance of the National TB Elimination Programme - Baramulla.

District Tuberculosis Officer - Baramulla Conducts Visits to Health Institutions across Medical Block Sopore - Baramulla.

The District Tuberculosis Officer (DTO) today embarked on a comprehensive visitation program to various health institutions within the district. The purpose of this visit was to assess the tuberculosis (TB) control and prevention measures in place, as well as to ensure the effective functioning of healthcare facilities with respect to Tuberculosis.

Accompanied by STS - HQs, the DTO visited a range of health institutions of PHC Tujar Sharif and PHC Bomai Sopore. The tour to rural areas, aiming to provide a comprehensive overview of the various Initiatives like PMTBMBA and TB Mukt Panchayats in district's healthcare infrastructure

During the visits, the DTO closely observed the implementation of TB Elimination programme, treatment protocols, and diagnostic practices. DTO Baramulla checked availability of necessary lab equipment, ATT Drugs, Treatment Cards and the skill level of healthcare personnel involved in TB management.

Additionally, the DTO actively engaged with the staff members, exchanging knowledge and discussing best practices for TB diagnosis, treatment, and prevention. The aim was to enhance their understanding of the latest advancements in tuberculosis management, emphasizing the importance of early detection and prompt treatment to prevent the spread of the disease.

A dedicated team of experts from National Health Mission, J&K and National TB Elimination Programme Kashmir embarked on a joint supportive supervision visit to District Baramulla to oversee the implementation of the National Tuberculosis Elimination Programme. The visit, which took place on 18th and 19th May 2023, aimed to assess the progress, identify challenges, and provide guidance for effective TB elimination measures.

The National TB Elimination Programme, a nationwide initiative, strives to eliminate tuberculosis as a public health problem by implementing evidence-based interventions and ensuring quality care for all TB patients. To ensure the program's success, regular supportive supervision visits are conducted to monitor the implementation at the grassroots level and offer support to healthcare providers and stakeholders.

The joint team comprised experts from the National Health Mission and National TB Elimination Programme, who brought their diverse experiences and perspectives to strengthen the district's TB control efforts. Together, they observed various aspects of the program, including diagnosis, treatment, monitoring, and overall healthcare infrastructure.

During the visit, the team engaged in interactive sessions with healthcare providers, program managers, laboratory personnel, community health workers, and other key stakeholders involved in TB control. These discussions facilitated the exchange of knowledge, identification of bottlenecks, and the development of strategies to address challenges faced in the district.

The joint supportive supervision team expressed their appreciation for the dedication and hard work demonstrated by healthcare providers and program implementers in Baramulla. They acknowledged the progress made in implementing the NTEP and commended the district's commitment to achieving the program's objectives.

While recognizing the achievements, the team also highlighted areas that require further attention. These included strengthening laboratory services, ensuring timely diagnosis and treatment initiation, improving the quality of data management systems, and enhancing community engagement and awareness.

The team provided valuable recommendations and technical support to address the identified gaps and enhance the effectiveness of the Programme in Baramulla. They emphasized the importance of collaboration, training, and ongoing supervision to sustain and scale up the efforts for TB Elimination.

The joint supportive supervision visit to Baramulla concluded on a positive note, with both the visiting team and the district health authorities expressing their commitment to work together in achieving the goal of a TB-free Baramulla. They acknowledged the significance of such collaborative efforts in eliminating tuberculosis as a major public health concern. They visited District TB Centre - Sopore, SDH Sopore, HWC Nowpora, HWC Delina, SDH Kreeri, SDH Kunzer and various Tuberculosis Units and Designated Microscopy Centres.

It is pertinent to mention the team was headed by Dr. Imtiyaz Ahmad Baba - Assistant Programme Manager - NHM, J&K.

NIKSHAY DIWAS - MAY 2023

Today, on May 6th, Nikshay Diwas was celebrated across Baramulla and Bandipora. The day was observed to raise awareness about the importance of early detection and timely treatment of tuberculosis (TB), which remains a major public health challenge in the country.

The theme for this month's Nikshay Diwas is "TB Preventive Treatment Services". This theme highlights the importance of preventive treatment for individuals who are at high risk of developing TB, such as those with HIV, diabetes, and other co-morbidities, as well as close contacts of TB patients.

The National Tuberculosis Elimination Program (NTEP) - Baramulla/Bandipora has been working tirelessly to eliminate TB from the Districts. Through initiatives such as Nikshay Diwas, the NTEP - Baramulla aims to create awareness about the disease and promote early detection and treatment.

All the HWCs of the District raised awareness about TPT and advocated TB is curable, and with timely diagnosis and proper treatment, patients can be cured of the disease and also concerted efforts are needed to eliminate this disease.

On this Nikshay Diwas, HWC Staff along with PRIs and Community pledged to do their part in the fight against TB.
