Most back pain is mechanical and gets better. A small amount isn't, and those cases need a doctor the same day rather than a physiotherapist.

Loss of bladder or bowel control. Numbness around the groin or inner thighs. Sudden weakness in a leg or foot. Back pain with fever, or after a fall.

What actually happens in your first physiotherapy session?

Mostly, we listen.

Roughly half of a first appointment is conversation, because what you can no longer do tells us far more than where it hurts. Then we watch you move, we test range and strength and balance properly rather than eyeballing it, and we explain what we found in plain words.

You should leave with three things: what we think is going on, what we're going to do about it, and roughly how long it should take. In writing.

What it isn't: a massage, ten minutes on a machine, or something done to you. Most of the work happens between appointments.

Your first four weeks at APARC โ€” week by week, honestly. Including the week where it feels like nothing is happening.

Week one. A long conversation, a proper assessment, and baseline measurements written down. You leave with a plan you can read.

Week two. The routine settles in. The symptoms often haven't changed yet. This is normal, it is expected, and it is the single most common point at which people quietly stop coming. Tissue capacity does not move in a fortnight.

Week three. We measure again โ€” the same tests, so the comparison means something. Numbers move before feelings do. If they've moved, the plan gets harder. If they haven't, the plan changes.

Week four. Most people feel better by now. That's the halfway point, not the end โ€” symptoms settle well before capacity returns, and stopping here is how a problem comes back in four months. We'll tell you when you're actually done.

And if nothing has changed after four weeks? That's information, not failure. We re-examine the problem rather than pushing harder at the same plan โ€” sometimes that means a different approach, sometimes it means referring you on. We'll say so.

"Why is my back worse in the morning?"

Because your discs have been drinking all night.

Lying down takes the load off your spine, and the discs quietly draw fluid back in. You genuinely wake up taller than you went to bed โ€” and stiffer, because a full, taut disc bends less willingly than a compressed one. By evening the same disc is flatter and more compliant.

That's mechanics. It isn't damage.

Three things for tomorrow: move before you judge it โ€” give your back twenty minutes before deciding how bad today is. Don't test it the second you sit up; bending fully forward first thing is the worst possible moment to try. And warmth, then movement, in that order.

Stiffness that eases as you move is a good sign. That pattern usually responds to loading rather than resting.

But slide 6 matters: morning stiffness that lasts well over an hour, for months, is a different problem โ€” and it's a medical one.

How good is your balance? This simple check may help you notice whether balance feels steadyโ€”but it is a screening activity, not a diagnosis.

Stand beside a stable support, look ahead and gently lift one foot only if it is safe for you. Notice excessive swaying, trunk leaning, repeated foot touching or the need to hold on.

Balance is a team effort involving vision, the inner ear, sensation from the feet and joints, brain processing and muscular responses.

If balance is worsening or is accompanied by dizziness, weakness or repeated falls, seek a proper clinical assessment. Do not try this alone if you are at risk of falling.

Today is World Physiotherapy Day.

Most people meet a physiotherapist on one of the worst days of their life. After the stroke. After the accident. After the diagnosis that changed the plan.

Nobody arrives happy. They arrive frightened, and usually holding a piece of paper they don't fully understand.

The work is everything that comes after. The first unaided step. The first meal eaten with the affected hand. The morning someone gets themselves out of bed without calling for help โ€” and doesn't mention it, because it has stopped being remarkable.

That is the job. It is slow, it is repetitive, and almost none of it looks impressive from the outside.

To every physiotherapist across our 12 centres in Delhi NCR and Pune โ€” thank you for the part of the work nobody sees.

And to everyone who has done the exercises on a day they did not want to: that was the hard bit. We know.
