My few observations on airway management of covid or any other suspect
( Based on inputs from many friends presently handling covid patients)

ALL MEASURES ARE MEANT TO MINIMIZE VIRAL LOAD EXPOSURE

1. Don't use stethoscope: clinically observe chest rise, fix tube upto 20-22 as we do conventionally.
If possible order an x ray to confirm tube position.

2. Always paralyze patients before intubation. One cough will expose you to a very high viral load.

3. Don't give bag mask ventilation, avoid high flow nasal oxygen, don't ventilate with open circuit ( Bains, ambu etc)

4. Clamp the tube,Don't let patient breath through tube in open air, immediately connect circuit, then remove clamp.
Alternatively put an HME filter on tube.
