Do you love helping people, building community connections, and making a real difference every day? We're looking for a Social Prescribing Link Worker & Health and Wellbeing Worker to join the fantastic team at Truro Primary Care Network.
💼 Full-time (37.5 hours per week)
💷 Starting rate: £14.50 per hour
📍Working across the Truro area (home & community based)
This is a unique blended role where you'll spend:
🏠 2 days a week supporting people remotely as a Social Prescribing Link Worker, helping them access services, activities and support that can improve their wellbeing.
🤝 3 days a week working face-to-face in the community as a Health and Wellbeing Worker, helping people build confidence, set goals and take positive steps towards a healthier, happier life.
We're looking for someone who is:
✅ Passionate about people and communities
✅ A great listener and communicator
✅ Friendly, approachable and organised
✅ Able to build trust and positive relationships
✅ Committed to helping people discover their strengths
Experience in health, wellbeing, social prescribing, coaching or community work is helpful, but we're just as interested in the values, skills and enthusiasm you can bring.
