If you support a family member, friend or someone you care for, Proxy Access allows you to use the NHS App on their behalf.

✅ Book appointments
✅ Order prescriptions
✅ View health information (with permission)

The level of access can be tailored to individual needs, and both people must be registered at the same GP practice.

Learning Disability Hospital Support 💙

If you or someone you care for has a learning disability and is attending a hospital appointment or staying in hospital, support is available from the Learning Disability Nurse Team at University Hospitals Tees.

The team can provide advice, support and help to make hospital visits and admissions easier for both adults and children, whether attending an out-patient appointment or being admitted as an in-patient.

🙂 Need a bit of help to stay safe and comfortable at home? The Home Improvement Agency (HIA) is here to support residents to live independently with home adaptations.

🫶 It’s available for people with disabilities, care and support needs, those aged 60+, or anyone at higher risk of falls.

🔨 The Handyperson service can also help with small jobs around the home — including two hours of free labour for eligible residents (excluding gardening and decorating).

🔧 From minor repairs to installing items like handrails or bannisters, we can help make your home safer.

Are you a carer looking for some time to relax, recharge and connect with others? Join the Stockton-on-Tees Adult Carers' Support Service friendly Monthly Gardening Group at the LiveWell Hub.

🌿 Enjoy gentle gardening activities
☕ Meet other carers in a supportive environment
🌞 Spend time outdoors and focus on your wellbeing
