Patchwork is now the system for submitting exception reports, following the change introduced on 1st April 2026.
All new exception reports must be submitted via Patchwork, as the previous Allocate system is no longer in use.
If you’re currently on placement in a GP practice, hospice or community setting, you should have received your Patchwork login details ahead of your go-live date. If not, or if you need support, please contact our Helpdesk Team.
Please note: Resident doctors in hospital-based placements should continue to submit exception reports via their Host Trust’s reporting system.
