Kae faced a spiral in her mental health after years of struggling, until the support of a neighbourhood team of professionals rallied around her.

After contacting her GP surgery, Kae was referred into a neighbourhood-based approach that brought together her GP, a care coordinator and other professionals.

Her GP, care coordinator Claire Edwards from ABC Primary Care Network and other NHS professionals worked together, staying in touch, sharing information and adapting support as Kae's needs changed.

Her story shows the difference coordinated care can make when professionals work together around the person, not just the problem.

"I stopped sleeping," Kae said. "The exhaustion made everything worse. I was crying all the time. Meetings made me physically sick. I couldn't function."

Kae contacted her GP surgery and, instead of a single appointment followed by a long wait, things moved quickly. She was referred into a neighbourhood-based approach that brought together her GP, a care coordinator and other professionals.

Her GP, care coordinator Claire Edwards from ABC Primary Care Network and other NHS professionals worked together, staying in touch, sharing information and adapting support as Kae's needs changed.

"It wasn't just one appointment and then being sent away," Kae said. "They stayed in touch. They checked in. They noticed when things weren't improving."

For Claire, coordinated care is about understanding the whole person, not just a single condition.

"We take time to understand each patient's full story and coordinate the support," she said.

Today, Kae feels like herself again: "I feel capable again. I've got my confidence back."

Her story shows the difference coordinated care can make when professionals work together around the person, not just the problem.

Lung cancer screening is launching in Maidstone and Tonbridge Wells soon 💙

The programme is aimed at current and former smokers, aged between 55 and 74 and you will be contacted if you are eligible.

Our lung cancer screening team have been at Tunbridge Wells Hospital today talking to patients and visitors as we get ready for the launch of our screening programme later this month.

🫁 The programme will offer lung health checks (and CT scans where needed) to eligible people aged 55–74 who have ever smoked.

This aims to detect lung cancer earlier and improve outcomes for our communities.

Those eligible for the screening programme will be contacted via their GP once the service starts.
