Live Well Hull A one-stop shop for people to find information, advice and support for adults in Hull!

Connect to Support has been developed by the Yorkshire and Humber regional councils to meet new care and support legislation in line with the personalisation agenda. The foundations of Connect to Support have been centred upon the experiences and expectations of our customers and deliver upon customer 'I' statements as set out in Think Local Act Personal 'Making it Real'. nables individuals and carers to have greater choice and control over the care and support they receive through bringing all available services, activities and information together in one place. It enables use of self funding, personal budgets and direct payments so individuals are in control of their own money and what it is spent on. It provides a local directory of free/low cost groups, services and activities to promote independence, market provision and community participation. Connect to Support is designed to enable individuals to live as well as possible, reducing dependency and reliance on formal care whilst promoting positive risk and safety. It is a useful information and advice resource for everybody to use and promotes greater understanding, delivers on customer expectations and enables collaborative working to improve the health and well-being of people in our city.

Stress, low mood, trauma, loss or physical health problems can all affect how we care for ourselves and our home.

We want to better understand people's experiences of self-care and self-neglect, so local services can respond with the right support.
