A new Health Action Plan from HPFT is helping community nursing teams in Essex provide more personalised care for people with a learning disability.

Co-produced with service users and staff, the accessible EasyRead plan brings key health information together in one place. It can be completed during appointments and shared straight away, helping service users, carers and professionals stay informed and involved in decisions about care.

Feedback from service users and carers has been positive, with many valuing receiving a clear written record on the day.

Well done to everyone involved and their commitment to improving physical health and making services more accessible for everyone.

We’re proud to share that our rehabilitation services have been rated Outstanding by the Care Quality Commission (CQC).

This recognises the exceptional care and support our teams provide to people with severe and complex mental health needs and marks the third consecutive inspection this year where services have improved their rating.

Our rehabilitation wards support people to rebuild confidence, develop independence and return to life at home following a hospital stay. Feedback from service users and families played a key role in the inspection, with many sharing the difference the service makes.

People told inspectors they feel safe, supported and genuinely cared for. One service user described the ward as a “home from home”, while carers spoke about the patience and kindness of staff and their focus on helping people gain greater independence.

Karen Taylor, Chief Executive at HPFT, said:

“The outstanding rating reflects the high-quality day-to-day care, compassion and commitment our teams bring. I’m really proud of the difference all our staff make every day for those they support.”

This week is Carer’s Week and it is an important opportunity to recognise the vital role that unpaid carers play every day.

Across our communities, thousands of people support family, friends and neighbours to stay well and independent.

At HPFT, carers’ voices are helping to shape how we involve and support them. This includes achieving re-accreditation for the Triangle of Care, a nationally recognised framework that helps services work more effectively with carers, and introducing an improved carers assessment, helping carers reflect on their own needs and access support to continue in their caring role.
