Surrey Heath PCN - Primary Care Network Surrey Heath PCN was formed by & is part of 7 GP Practices in Surrey Heath & Ash Vale

The PCN was formed in response to a national ask made to all GP Practices in England. PCN’s enable GP Practices to work together, locally, to collaborate on the recruitment of additional workforce, to support resilience and to support the delivery of additional services to their patient populations. Today is Young Carers Action Day - Young Carers Action Day provides an opportunity for everyone to call for better support for young carers and young adult carers, and to bring about change.

This year's theme is fair futures for young carers. The day will focus on building a more equal future for young carers and removing barriers that many of their peers do not face.

- 2 in 3 smokers will die as a result of long-term smoking.
- 1 in 4 hospital patients is a smoker.
- Every day someone is admitted to hospital with a smoking-related disease.
- In 2022-23, there was an estimated 400,000 hospital admissions due to smoking.
- It’s estimated nearly 50 million ci******es are smoked every day – around 9 ci******es a day per smoker.
