Our Home from Hospital Service offers FREE, short-term support to help you settle back in safely and comfortably after a stay in hospital.

Whether it’s a helping hand with light meals, picking up prescriptions, or simply making sure your home is warm and welcoming, we’re here when you need us most.

We’ll support you for up to six weeks, helping you regain confidence, stay independent, and get back to your normal routine 🏡

✅ Friendly “meet and greet” when you arrive home
✅ Help with shopping, paperwork, and appointments
✅ Regular pop-in visits for reassurance
✅ Support to find other services if needed

Many families worry about parents or relatives who are living independently but may be at risk of falls, fires, wandering, seizures, or not being able to call for help.

Durham County Council’s Telecare service, provided by Care Connect, uses discreet sensors and technology to help keep people safe at home – while protecting their independence.
