Health and Safety Executive The Health and Safety Executive (HSE) is Britain’s national regulator for workplace health and safety.

We aim to reduce work-related death, injury and ill health in Great Britain. To help us do this, we carry out research, provide information and advice, promote training and propose regulations and codes of practice. Our inspections have found that RPE failures are the most widespread theme across bodyshops – and they're rarely down to one thing.

Inspectors are repeatedly finding a chain of problems: incorrect mask types, poor face-fit, facial hair preventing a seal, damaged or incorrectly stored equipment, and RPE simply not being worn consistently – even when it's provided.

Filtering respirators do not protect against isocyanates. Only air-fed RPE certified to BS EN 14594:2018 with a low-flow warning device provides adequate protection.

Having the right kit in the cupboard is not enough. It needs to be worn correctly, maintained, fitted, and used every single time.

“It breaks my heart that Adam will never chase after his grandchildren again.”

A Manchester construction company has been fined after joiner Adam Kirkpatrick suffered life-changing injuries when he fell through an unsecured skylight opening while carrying out roof work on a domestic property in Altrincham.

Adam suffered a complete spinal cord injury and is now mostly housebound.

An HSE investigation found the principal contractor failed to properly plan, manage and monitor the roof work and did not ensure suitable measures were in place to prevent falls from height.

The company was fined £8,000 following HSE’s prosecution.

A yard operative in Aberdeen suffered life-changing injuries after being struck by a reversing forklift truck.

The worker was unclipping a trailer curtain at Streamline Shipping Agencies Limited's premises when the forklift's rear wheel made contact with his right leg, causing multiple fractures, a serious de-gloving injury, and requiring two surgeries and skin grafts.

Our investigation found the company's traffic management plan lacked the detail needed to keep pedestrians safe where loading and unloading happened simultaneously. Streamline was fined £146,700 at Aberdeen Sheriff Court on 22 May 2026.

A plan on paper is not enough - it must genuinely protect workers on the ground.

A worker lost his life. It didn't have to happen.

Stuart Garnet, 44, was fatally struck by a reversing telehandler at a Cumbria waste and recycling facility in 2022. This week, H. Wicks (Lindal) Limited were fined after failing to ensure pedestrians and vehicles could circulate safely on site.

HSE Principal Inspector Caroline Shorrock said: "Had the company implemented suitable separation measures, this fatal injury would not have occurred."

A ship’s engineer suffered serious cold burns after an estimated 250–300 kilograms of liquid propane was released at pressure in a matter of seconds.

Vladimir Volkov, a gas engineer aboard the tanker MV Symi, sustained cold burns to 10–13% of his body after liquid propane was released without warning.

The incident happened in the early hours of the morning at Braefoot Bay Marine Terminal in Fife in November 2018.

Our investigation found Shell UK’s flawed safety procedures led to the violent release of liquid propane during loading operations at the marine terminal in the Firth of Forth.

The incident created a rapidly expanding flammable vapour cloud that enveloped workers on both the ship’s deck and the adjacent jetty.

It could have resulted in a catastrophic explosion if the cloud had found an ignition source.

The energy giant was fined £450,000 at Kirkcaldy Sheriff Court on 26 May 2026.
