🏥Sports Participation Physicals at USAHC Stuttgart – Limited Slots Available!
🗓️Dates: June 25 & July 10, 2025
📍Location: U.S. Army Health Clinic Stuttgart, Patch Barracks
🕰️Time: 1:00 p.m. – 3:30 p.m.
📋 Appointments: Required (no walk ins & each child requires their own appointment)

What to Bring:
*Up-to-date vaccination records (including the German Yellow Card, if available)
* Health Assessment/Sports Physical Statement (HASPS) form (page 1 filled out)
*Glasses (if used)

🚧Note: Immunizations will not be provided during these sessions. If a CYS Services SNAP Allergy Medical Action Plan is required, these must be completed through your respective Host Nation Primary Care Provider who normally prescribes rescue medications.

📑Please visit the Medical Records desk Mon-Friday 7:30am – 1:00pm to pre-register and verify current health insurance information. This visit is not free and will be billed to your health insurance as an annual WELL Child visit. Once registration is complete, you will be assisted with scheduling your time slots and provided all required paperwork.

The AHC Stuttgart Pharmacy will be closed during lunch next week, 8-12 June 2026, closing from 12:00pm to 1:00pm due to staffing. PLEASE NOTE the ability to pull a kiosk ticket stops at 11:30 and re-opens at 12:45. Please plan ahead when visiting the clinic for services during this time.

Per HQDA EXORD 202-24 and USAREUR-AF FRAGO 38, TIER 1 personnel will need ANAM testing completed before the September 30th, 2025 deadline, TIER 2 before the September 30th, 2026 deadline, and TIER 3 before the September 30th, 2027 deadline. Please see attached for TIER explanation.

The ANAM is a 40-45 minute computer-based cognitive assessment, as you may have completed at an SRP. Tier 1 personnel will need to test annually under the new DOD Cognitive Monitoring/Blast Overpressure Program.

TIER 1 - Annual Assessment
TIER 2 - Every other year
TIER 3 - Every 3 years

Service Members in the Army, Navy, and USMC can:
✅ Complete the online PHA Part 1 in advance
✅ Check in at the clinic for record screening
✅ Complete all overdue services (HIV, Hearing, Immunizations, etc)
✅ Schedule follow to see your PCM (virtual or face-to-face)
